As a Lions Foundation of Canada (LFC) Chair, you play an important role in supporting communication and engagement between Lions Foundation of Canada Dog Guides (LFCDG) and Lions within your district.

To help strengthen communication between LFC Chairs and LFCDG Ambassadors, LFCDG is requesting your consent to share your contact information with our current Ambassadors.

Contact Information Sharing

I give Lions Foundation of Canada Dog Guides permission to share my:

· Name

· LFC Chair role/ District

· Email address

with current LFCDG Ambassadors for the purpose of facilitating communication and collaboration related to LFCDG activities, initiatives, presentations, outreach, and Lions engagement.

This information may be included on an internal LFC Chair contact list provided to LFCDG Ambassadors. Contact information is intended for LFCDG/Lions Foundation-related purposes only and is not intended for public distribution.

Consent

I consent to LFCDG sharing my name, district, role, and email address with LFCDG Ambassadors as described above.

I understand how my information will be used and shared as outlined above. I understand that I may contact Lions Foundation of Canada Dog Guides if I wish to update or withdraw my consent for future sharing.
