Autism Assistance

Autism Assistance Dog Guides help children between 3 and 12 who are on the autism spectrum. These Dog Guides provide safety, companionship, and unconditional love. They offer a calming relief for children in high anxiety situations, and reduce the stress commonly experienced in public places.

In order to apply for an Autism Assistance Dog Guide, you must submit both the Program Application and Medical Report form.  Both forms must be completed and submitted in order for us to accept your application and place your family on our waiting list.  The forms are available for download by pressing the links below, if you require the forms in an alternate format please e-mail Breanna in our Client Services department.

Please Note

The Autism Assistance Dog Guide program is now accepting new applications.  Once you have submitted your completed application, you will receive an e-mail confirming our receipt of it.  It will then take up to 4 weeks to fully process your application, enter your families name on our wait list, and send out an e-mail detailing the next steps in the process.  Applications will be processed in the order that we receive them in full.

Due to the past success of the program and high volume of past applicants, we will be accepting the first 100 complete new applications.  At that time we will temporarily close the application list, until we are able to evaluate and meet the needs of the families on our waiting list.
